The Government of Himachal Pradesh, India, undertook a comprehensive initiative to conduct mass screening of its population aged 30 and above for non-communicable disease (NCD) risk factors. To streamline this extensive process, a digital platform was developed to facilitate data entry at the sub-center level. The state provisioned Android tablets to all Auxiliary Nurse Midwives (ANMs), enabling them to collect detailed information for health cards as well as other program interventions.

The application offers several functionalities to support the screening exercise, including:

• Providing supportive supervision and progress reporting across different levels of the administrative hierarchy
• Estimating disease prevalence based on administrative geography
• Assessing the proportion of individuals living with NCD risk factors
• Enabling public access to health status information through secure OTP verification, thereby promoting health awareness
• Implementing an automated SMS alert system for follow-up with individuals identified as suspected or confirmed with NCD risk factors
• Offering both online and offline modes via Android mobile applications, as well as web access for relevant stakeholders
• Ensuring the availability of tablets and a flexible health-card application to support evidence-based decision making and enhance program efficiency

The application's adaptable data architecture allows for the integration and monitoring of additional health indicators beyond the current NCD metrics, ensuring scalability and comprehensive health data management.
